I. SHPDA Administrative Report -

AL2015-024, Greystone Surgery Center LLC, d/b/a Greystone Surgery Center, Birmingham, AL: Proposes to establish an Ambulatory Surgical Center through the conversion of an existing surgical center originally constructed as part of a physician’s private practice. The project was approved with limiting language by order of the administrative law judge. No vote is required because the recommended order of the administrative law judge becomes the final order of the agency when no exceptions have been filed.

A. Project Modification AL2013-059, Renal Treatment Centers-Southeast LP, d/b/a East Jefferson Dialysis: Proposes to establish an end stage renal disease (ESRD) facility consisting of two existing hemodialysis stations to be relocated from DaVita’s Birmingham East Dialysis, plus add eight new hemodialysis stations.
