DMV DUI Reinstatements & Interlock In Colorado Springs

If you have either refused a chemical test or have a blood alcohol concentration (BAC) above .150, there are additional requirements for reinstatement of your driving privileges. You must enroll in Level II alcohol education and obtain SR-22 insurance. To initiate the reinstatement process, you need to complete four forms, which I have included links for. These forms, along with a $95.00 check and proof of SR-22 insurance, must be submitted to the DMV. You have two options for submission: you can either mail the forms to the address specified on the Application for Reinstatement, or for a faster alternative, you can utilize Colorado's myDMV online platform. On the myDMV website, click on "Driver/ID Services," and under "Additional Services," you will find links for paying the reinstatement fee and uploading documents. By selecting the payment link, you can pay the $95.00 reinstatement fee, and then proceed to upload the required documents by clicking on the "view/upload documents" link. Simply scan and upload the attached documents through this online portal. This option provides a quicker and more efficient means of submitting the necessary documents to the DMV.

Colorado Springs DMV Forms & How To Properly Fill Them Out

To begin the reinstatement process, you will need to complete several forms. The first form is the Application for Reinstatement, where you should fill in all the requested information. For the Alcohol Certification form, complete the top section with the required details, then initial each necessary section and sign the form. Moving on to the Affidavit of Enrollment, fill out the top portion and take the document to the agency where you will be undergoing Level II education. Ensure that someone from the treatment provider signs the document in the designated field. Lastly, for the Restricted License Agreement, it is essential to have the interlock device installed in your vehicle before submitting the reinstatement packet. Colorado has four approved interlock device providers, and their contact information can be found below.
