Minnesota Delegation of Parental Authority

This Minnesota Parental Delegation is a form for Minnesota parents or guardians to delegate parental authority to another person. This form is best used whenever a parent must be away from his or her children for any extended period of time; for example, a vacation or military duty or a business trip. The form allows the children's temporary care-giver, like Grand-Parents, to "step" into the absent parent's shoes and act as a parent in all things. This form is especially useful in the event of a medical emergency and neither parent is available. The form is posted here for personal use compliments of Kate Willmore.
