OSHA PPE Hazard Assessment Certification Form


OSHA requires that all employers certify the personal protective equipment required for each job at an establishment. Although OSHA requires only the job title, PPE listing, and signature be written, a hazard assessment must be performed. This form combines the elements of a hazard assessment and certification into one complete record.
