Comprehensive guidance on the topic of school health records can be found in Questions & Answers Regarding School Health Record Issues - May 2013. This Q&A document, developed as a joint effort by the Illinois State Board of Education (ISBE), Illinois Department of Human Services (IDHS) and Illinois Department of Public Health (IDPH), contains answers to over 50 questions relating to student health records, ranging from immunizations to dental examinations. The guidance, which contains the names and contact information for various state consultants, will be a useful resource for school personnel.  

Of particular note for the 2013-2014 school year, ISBE will allow school districts to accept either of the two most recently revised physical forms (January 2012 and February 2013). ISBE had previously indicated that the 2013 form would be required this coming school year, but has decided to give school districts a one-year reprieve.
