This is a difficult to find Immigration Form that is used by aliens who receive voluntary departure from the Immigration Judge. This form is one piece of evidence that the alien left on or before the date that the Immigration Judge had provided for the voluntary departure. This form can be used as evidence that an alien complied with Immigration laws and also was outside the country for the requisite amount of time in the event that the alien was subject to a 3 or 10 year bar because of past immigration violations. The alien, in addition to completing this form, should also get some other type of proof that they departed on the date such as a credit card receipt from the country to which they departed or plane, bus or train ticket showing that they did leave the United States.
