Disease management (DM) is a process that evolved from managed care principles. It uses evidence-based medicine, along with practice guidelines to assure patients receive optimal care while providing a favorable return on investment. This paper will explore a proposed DM approach to chronic obstructive pulmonary disease (COPD) DM program. COPD is diagnosed via spirometry and treated with pulmonary rehabilitation. DM proposes to reduce the amount of respiratory flare-ups, shorten hospital stays and increase patient well-being.
