Requesting Medical Records of Deceased in Arizona


In Arizona a deceased patient's spouse, adult child, parent of deceased, adult sibling of deceased, or guardian/conservator may obtain the deceased patient's medical records. (ARS 12-2294)

A copy of the deceased patient's Death Certificate should also be enclosed.
