New York form of Power of Attorney


The attached document is a New York State form of Durable Power of Attorney. It must be completed correctly and in accordance with the wishes of the Principal(the person granting the right to another to act on his or her behalf).

The Principal and the Agent (also referred to as the Attorney in Fact) should consult an attorney to assist in its completion and proper signing. This form changes from time to time based on changes in the law. Hence, always consult an attorney to make sure you are using the latest legally acceptable version.
