AL2021-043, Ball HealthCare-West Mobile, LLC, Mobile, AL: Proposes to construct a seventy-five (75) bed skilled nursing facility, comprised of thirty-four (34) beds relocated from Spring Hill Manor Nursing Home and forty-one (41) beds relocated from Twin Oaks Rehabilitation & Healthcare, providing inpatient and outpatient rehabilitation services as part of a planned senior housing community. AL2021-047, Heritage Assisted Living and Memory Care, LLC, Madison, AL: Proposes to convert eight (8) existing licensed Assisted Living Facility (ALF) beds to Specialty Care Assisted Living Facility (SCALF) beds, adding them to the existing sixteen (16) SCALF beds already in service.
