The Occupational Safety and Health Administration (OSHA) requires most employers with 10 or more employees to track and report all work-related injuries and illnesses via Forms 300 (Log of Work-Related Injuries and Illnesses) and 300A (Summary of Work-Related Injuries and Illnesses). In order to complete Form 300A, employers must summarize all work-related injuries and illnesses reported on Form 300 for the 2019 calendar year, including the total number of cases, total number of days employees spent away from work or on restriction, and specific injury and illness types.

The reporting deadline for Form 300A is March 2, 2020. In addition to filing Form 300A, employers must post the form in the workplace from February 1 to April 30 annually. Form 300A may be electronically filed via OSHA’s Injury Tracking Application.
