The Office of Management and Budget has renewed the self-identification form relating to voluntary self-identification of disability for employees of federal contractors.  This form has an expiration date of January 31, 2020, and is administered by the Office of Federal Contract Compliance Programs (OFCCP).  Federal contractors are required to use the renewed form or update their electronic versions to reflect the new expiration date of January 31, 2020.  The form can be found on the OFCCP website and is available in multiple languages including English, Spanish and Chinese.

As federal contractors will remember, the OFCCP implemented multiple regulations regarding Section 503 of the Rehabilitation Act in 2013.  These regulations specifically relate to self-identification of disability and require federal contractors to annually document and update  several quantitative comparisons for the number of applications and hires. It further requires that Federal Contractors invite applicants, both pre and post offer, to self-identify with disabilities on a specific form provided by the OFCCP.  Contractors are also required to request employees to self identify every five years, again, using the prescribed form. A fact sheet for technical compliance can be found on the OFCCP website.
