Request AL2020-034, The Haven Memory Care Facility, Baldwin County, AL: The project modification request is for the approval to change the construction of twenty-four (24) new Specialty Care Assisted Living Facility (SCALF) beds to the conversion of twenty-four (24) of its existing single occupancy SCALF units to double occupancy units. This alternate plan has been reviewed and approved by the Alabama Department of Public Health (ADPH). The estimated cost of the room conversions is $75,000.
